DXF-wise (i.e. no ActiveX) you'd entmod the BlockRecord's 70 code: http://docs.autodesk.com/ACD/2013/ENU/index.html?url=files/GUID-A1FD1934-7EF5-4D35-A4B0-F8AE54A9A20A.htm,topicNumber=d30e675444

It uses the same value as the InsUnits sysvar: http://docs.autodesk.com/ACD/2010/ENU/AutoCAD%202010%20User%20Documentation/index.html?url=WS1a9193826455f5ffa23ce210c4a30acaf-4f8f.htm,topicNumber=d0e348840

So you'd be changing it from 1 to 4.

If you go the ActiveX route, then you can step through the active document's blocks collection. And set each of those Block definition objects' Units property:
http://entercad.ru/acadauto.en/index.html?page=idh_units.htm

Just take note that any inserts already placed might need to be scaled after this.

When you change your block's units, are you changing their size since now they are 25.4x smaller?

I copied and pasted block from one drawing to another and when I tried to insert one of these blocks I realized the big size of the block and closed the drawing without saving , after that I resized the block to millimeters with the help of Mr irneb and recopied and pasted the blocks again and it works great now .  :-)

Thank you .
