Had it happen to me this morning so I did some screenshots.

Image3.jpg is the 'xref has been modified, blah blah,' balloon notification.

Image4.jpg is the open file dialog.  A very common alternative to this one is that all file dialogs have "Save Drawing As %S", regardless of whether you are saving, opening, saving as(save asing?).

This issue is likely caused by a memory issue.
I had this problem on several workstations and increasing the memory finally resolved the issue

While reading the first posts to Keith's, I was leaning towards it being a memory problem also. I have also seen where a workstation has enough memory, but one of the chips has a problem with it. I switched the RAM with another of the same size and all worked well. So, I replaced the original RAM to rule out the RAM not being seated well and the problem re-appeared, thus Bad RAM.

CAD seems to use memory differently than other programs. The machine I am talking about ran all of its other programs fine.

Just something to think about,

How much RAM is in this box? How large is your swap file? And your video card is?
