help modifying a lisp routine

Hi,
Firstly i think this is an awsome site and source of information. thanks guys.

i have downloaded a lisp (not sure who authored it) routine that calcualtes the number of blocks selected and puts them into a table with the symbol alongside. However, the symbols ends up being very small and it only shows the block name and quantity.
Can someone help me to modify it a little to get the end result to look like the image attached.
basically i would like to extract the following information on my legend.
1. Block Name
2. Block Symbol
3. detailed description of block
4. quantity.
Is there anyone out there who is willing to make this work for me.
