Program/Work request form

Does anyone have some type of work request form they use when discussing a new program with a client? Some thing that lists requirements, objectives, user prompts, GUI/no GUI, etc....? If so can I see it? I'm thinking of making one up and I would like to see what someone else might use.

Nope, but that sounds like something that could be needed for those that need to explain the code they either want or are trying to write and just can't get the pseudo-code out of their head. I'd like to see it when you get it done.
