I use UCS > Object and PLAN > Current. Then ALWAYS return the ucs to WORLD.
I find this takes less thought than DView > TWist as I don't need to figure out the angle myself (Lazy I guess...)

I prefer to create a new UCS within the viewport...that way I can still ID a point and get correct coords on it. I know...my method has been poo-pooed before....but it works for me....and I will continue to use it.

Just takes a bit longer to set up.

... I try to keep the rotation in the model the same as in the viewports if I can. (and still return the UCS to WORLD when I am done)...

Really? When I ever I used that method my coordinate system went out the window, so to speak.

We use QLeaders on all 'floor plan' dwgs.

When we rotate View Ports, it also rotates the QLeaders - even NEW ones.

So we are stuck rotating the actual floor plan, which is something I would rather not do. I would prefer to leave them as we recieve them from other companies.

I use Qleaders with no problems in ACAD2k.
My elevations are rotated around the floor plan. I use my buttons to orient my view while working on them then in paperspace I rotate the VP via dview twist. All my leaders are in model space.

Say I take a floor plan from an outside source that show has the North to the Right.

I paste it into MY title block with North to the right.

I then click on my Layout tab and go into the Viewport. Now when I rotate that Viewport so that North is Up, then create my QLeaders, the QLeaders are all sideways.

Am I doing this wrong?

After you rotate your viewport, set the UCS to view.

This is the button I use
[UCS Left]^C^Cdview;;tw;90;;_ucs;w;_ucs;z;270;
Then add my leaders in Model space.

I go to the tab & rotate my VP and all is well.

I scale all of my QLeaders to Match .1 text size when viewed in Paperspace, So I HAVE to put them in while in the Viewport.

Another thing, when you put your QLeaders in while in Model Space, how do you know that they are entirely viewable in the Viewport other than switching back and forth?

That method of adding the leaders my be your problem with the orientation.

If the leaders run outside the VP I enlarge the VP or move the leader or move the modelspace.
You can also create an outline in MS matching the VP outline. There is a LISP for that. :)

To keep your text sizes correct across multiple VPs, try setting the text to annotative. That way it can be .1" regardless of the scale of the VP its in.
