I know that perhaps no one can answer this question, but just curious...
Which centroid point is more accurate?
Look in DWG to see the problem...

Hi,
If you extrude the spline and get the centroid of the newly created 3d solid, the projection of this centroid on the XY plane is quite on the point got with gcen osnap (1e-14 tolerance).

You are right, but on AutoCAD 2017 where gcen exist... On A2014, projection of centroid don't match to either point... This is what I get on A2014... (attachment)

Autodesk should have change the way they compute a (extruded or not) Spline centroid when they added the gcen osnap with 2016 version.
Anyway, a spline is an approximated curve which necessarily alters the accuracy of the calculations.
